Most of history has had extreme wealth inequality without revolution. Most fighting has been between elites, not elites vs commoners. The rare instances of commoner uprisings has almost always resulted in commoners being massacred.
This is not true for post industrial society. Most of them had the second tier replacing the elites and becoming elites themselves.
