Thanks for the detailed write up. I did get the email saying my enrollment was accepted and I did accept the "Apple Developer Program License Agreement":

> Your enrollment request has been accepted. To complete your organization's enrollment in the Apple Developer Program, please sign in to your developer account to review and accept the Apple Developer Program License Agreement, and complete your membership purchase.

It's really only an issue when it's time to pay for the membership. I'm probably tripping some fraud detection system tied to Apple Store rather than Apple Developer. I'm traveling right now and don't have an iPhone with me but I'll try your suggestions when I get back after tomorrow.
